W.E.F 18th August 2014m Consulate/VFS require new visa application form for Tourist and Business category. Please download form as per below, Child below 18 yrs has to submit his Birth Certificate copy along with application and need to carry original with true copy at the time of Travel if Traveling alone or with parents.

Business Visa :
Valid passport last 6 months,
One visa application form No BI-84 duly filled and signed by PAX by black pen only as per the ppt,
02 natural recent photos 35mm x 45mm white background 80% face must be seen not older then one month, teeth must not visible mouth closed and not used in any visa,
Covering letter on company letterhead with physical address and contact details addressing to the South Africa consulate Mumbai with authorised person name, designation seal and sign mentioning travel dates with duration of stay with pax name and passport number,
Invitation must be addressing to consulate with full physical address contact details seal and sign with authorised person name and designation mentioning travel dates with duration of stay with pax name and passport number,
ID card copy of Authorized signatory person who signed the invitation letter from South AfricaHotel confirmation with physical address and contact details with applicant name arrival departure date,
If pax having own business then we required Personal & original company bank statement for last 03 months with bank original seal & signed.
If pax is working then we required Salary slips last 3 months & Salary accounts bank statement for last 3 months with original bank seal & signed .
Return ticket.
Tourist Visa :

Original leave letter on company letter head with physical address and contact details addressing to the South Africa consulate Mumbai with pax name passport number leave dates as per the travel date with authorised person name, designation seal and sign, for children require NOC letter on school/college/university letter head with physical address and contact details with pax name passport number leave dates as per the travel date with authorised person name, designation seal and sign,
Hotel confirmation on south African hotel letter head with physical address and contact details with seal and sign of authorised person name and designation with applicant name and passport number arrival departure date,
If pax having own business then we required Personal & original company bank statement for last 03 months with bank original seal & signed.
If pax is working then we required Salary slips last 3 months & Salary accounts bank statement for last 3 months with original bank seal & signed .

Invitation must be addressing to consulate with full physical address, CTC number seal and sign with authorised person name and designation mentioning travel dates with duration of stay, 01st Job offer letter from Indian company clear and dark copy,
Original PCC from Indian police or passport office,
Original medical and radiology report with doctor seal and sign,
Salary slip for last 03 months,

Family and Friends visit :
Valid passport for 06 months,

Personal bank statement of sponsor for last 03 months with bank seal and sign, sponsor passport copy, residence permit copy or visa copy attested by South African Police or Home Affairs, Sponsor latest employment proof on company letter head with full physical address and contact details with seal and sign of authorised person with his name and designation,
